Why the contribution is important

For outpatients: goal is keep people well/ fewer hospital bed days means cost savings for health system/ mental well-being if patients can be at home/data collected over time for patients allows detection of "abnormal for them" triggering face-to-face review

Also some inpatients can be monitored remotely once the model of care is set up.

    This sounds really interesting. I wonder what the patient journey would look like for someone with COPD in Metro South Health? What are your thoughts?

    I think it is really important to understand how virtual care models can complement face to face and other interactions. Maybe some pathways could be developed to help us understand this. COPD could be a starting point.

    I feel like this would be a great idea to monitor at risk patients for a period of time after they discharge from hospital as the home environment could present new challenges that were not considered at time of discharge.

    Regularly checking in with the patient to see if there are any small issues would be a great way to prevent larger, more serious issues from developing resulting in a return back to the hospital.

    Also, having a trained professional checking in would also would ease the transition into home by reducing a significant amount of uncertainty and stress, possibly allowing patients to be more confident to leave hospital sooner.

    This is like our HITH model. we need allied health to make this care and monitoring comprehensive. Need to change the attitudes of the public and medical staff re the safety of discharge home. Telehealth a great asset for this model.

    This model of care is well worth exploring. There are many patients who could be monitored at home via this model of care. Great idea.
