using community groups and volunteers to aid early re-engagement of patients in the community

why aren't we engaging with volunteer community groups to use this resource to get patients home with support and assistance?  with appropriate screening of group members this could be a resource alternative/conjunct to the not for profit organisations in the community that also provide assistance with personal care, transport, meals and shopping assistance

Why the contribution is important

purposeful engagement of community members is a valuable resource and is economical and has personal meaning for those contributing and receiving this assistance

Could be great. I now there are organisations in some cities facilitating people staying in their home longer by pairing them with younger people who are struggling to find affordable accommodation. They assist their elderly host in return for cheap or free rent.

Leisure Therapists do this in their job. There is just not enough of them resourced for all the patients who could benefit from their interventions.

Also, Volunteer Services in hospitals could be resourced better to have Relationship Managers to create community partnerships etc.
