Use of risk calculators to inform surgical decision making and discharge planning

Promote the use of NSQIP risk calculator amongst surgeons, anaesthetists and multidisciplinary teams involved with surgical/non-surgical decision making, to make predictions about length of stay and probability of requiring discharge to a nursing or rehabilitation facility.  These predicitions could inform decision making, triage and discharge planning about semi-elective and elective surgery.

The risk calculator is reasonably well validated and NSW Health are currently making a contribution, to what is currently largely North American data set.

Why the contribution is important

The NSQIP predicitions could inform decision making, triage and discharge planning about semi-elective and elective surgery.  For example if the hopsital is currently close to capacity, non-urgent or semi-elective surgery, on a patient with a high attendent risk of discharge to nursing home, could be deferred, pending a more favourable hospital capacity situation or improved discharged planning situation.  The benefit to be realised here, concerns improved management of acute hospital beds, informed by the likelihood of prolonged admission post-operatively.
